1.  Checks/Money Orders

Please make your check/money order payable to JCADF and mail them to:

2.  Credit Cards

You can make a credit card donation in 2 ways:

a.  Print

Click the image at right to print out a credit card form.  Please complete it and return it to the above address or email it to krock@epix.net.  If you wish your donation to be recurring (i.e.  a set donation will be automatically charged to your credit card at the same time each month), please check the "recurring donation" box on the form.

b.  Online
